Sciatica — pain, numbness, tingling, or weakness radiating from the lower back through the buttock and down one or both legs — affects an estimated 40% of people at some point in their lives. It is caused by compression or irritation of the sciatic nerve, most commonly through lumbar disc herniation, piriformis syndrome, spinal stenosis, or degenerative disc disease.
Conventional treatment: NSAIDs, muscle relaxants, nerve blocks, physiotherapy, and in refractory cases, surgery. Most sciatica resolves without surgery — but the path to resolution through conventional care alone is often long and painful.
Acupuncture for sciatica targets nerve root inflammation and central sensitisation simultaneously. Distal needling along the Bladder channel — points on the lower leg and foot — achieves immediate and sometimes dramatic decompression of sciatic symptoms. Electroacupuncture applied along the sciatic nerve pathway modulates nerve conduction, reduces inflammatory mediators around compressed nerve roots, and accelerates myelin repair. Local gluteal and lumbar points release piriformis spasm and paraspinal tension that perpetuate nerve compression.
In TCM, sciatica is understood as obstruction of the Bladder and Gallbladder channels through the lumbar, gluteal, and posterior leg regions — by Cold-Damp, Blood stasis, or Kidney deficiency failing to nourish the sinews.

What the Research Says
A meta-analysis in the Journal of Pain found acupuncture significantly superior to NSAIDs for sciatica pain intensity and functional disability. A Cochrane review of disc-related sciatica found acupuncture produced greater short and medium-term pain relief than conventional treatment.
